去文学部门拿一张同意书 consent form 她都要来个大转圈。我就耐心的看她，系主任的秘书如何处理。她拨电话到吉隆坡，然后，一个部门接一个部门的转来转去，最后，转回来我的这个部门。

我今天接到高教部的电子邮件，

Good Day! Mr. XXXXXX
Ms. H (FGO, FAS) told me you need a letter to do research. Perhaps you can forward the letter and see what I can offer my help.

我这样的回复她：

Saluton Ms YYYYY
No, she made an error, it is not the letter.
It is the consent form. You forwarded the mail to the undergraduate department last year to see whether there is a possible a format of it but i waited till this year and got nothing from your colleague.
Regrets that the FAS staff did not know what the consent form is. I am not the 1st PH D research student and there should be someone have done that. If it is not, the depeartment of FAS should have done it before taking the student.
I went to have discussion with Dr T on Tuesday and he told me that there is a form in the web 2, only the staff can assess it. The consent form on the IPRS site is for the science research. Dr T suggested not to create too many forms as each form has the code number and it might not be feasible for tracking. That is the reason i went to see the FAS staff for the possible tracking the form.
I need the consent form urgently as I need to send to the parents for signature.
